Answer every question below before you submit your packet. One "No" means a gap you need to fix. Do not submit with any unresolved "No."
Readiness Check
Have you confirmed your GDOT or MARTA portal status shows "Needs Reevaluation" and your contact info is current?
Have you gathered every item on the document checklist and confirmed that the business name, owner name, and address match exactly across all documents?
Does your narrative describe incidents within the three accepted categories: Education, Employment, or Business History?
Does your narrative use the When / Where / Who / What / Why / How structure for every incident?
Does your narrative avoid any mention of race, ethnicity, or sex as the basis for disadvantage?
Have you attached at least one piece of supporting evidence for each incident in your narrative?
Have you completed your PNW using the official USDOT form, excluded all non-reportable assets, and attached supporting documents for every number?
Do you have current proof that your business is active? (Active contracts, recent invoices, payroll records, or hauling tickets)
Are all insurance certificates, vehicle registrations, CDLs, and your equipment list current?
Has a qualified reviewer looked at your narrative and PNW statement, and have you addressed their feedback?
