Required Documents to Upload

Gather every item on this list before you write your personal narrative. Consistent documentation is the baseline — mismatched names, missing dates, or incomplete records are the top reasons packets get sent back.

Tip: Make sure your business name, owner name, and address match exactly across every document you submit.

1 Business Documents

  • Articles of incorporation, LLC paperwork, or partnership agreement (legal name must match all other documents)
  • EIN confirmation letter from the IRS
  • Current business address and contact information
  • Operating agreement, bylaws, or documents showing ownership and management control
  • Organizational chart showing ownership structure and key roles
  • Existing DBE certification letter from GDOT or MARTA
  • Any prior correspondence with GDOT or MARTA

2 Owner Documents

  • Government-issued photo ID for each owner (driver's license or passport)
  • Evidence of ownership: stock certificates, membership certificates, or share ledger
  • Personal tax returns for each owner — last 3 years
  • Resume or bio for each owner showing experience and day-to-day control of the business

3 Financial Documents

  • Business bank statements — last 12 months
  • Business tax returns — last 3 years
  • Current profit and loss (P&L) statement
  • Current balance sheet
  • Loan agreements and credit lines (not including your primary residence mortgage)
  • Accounts receivable and accounts payable ledgers

4 Trucking Operations Documents

  • Equipment list with VINs for every truck
  • Vehicle registrations and titles for each truck
  • Commercial Driver's Licenses (CDLs) for the owner and key drivers
  • Insurance certificates: commercial auto, general liability, workers' compensation
  • DOT inspection reports and applicable permits

5 Supporting Records

  • Copies of active contracts and subcontracts
  • Hauling tickets, invoices, and proof of payment showing your business is active
  • Lease agreements for property or equipment, if applicable
  • Required Georgia DBE recertification state forms
  • Evidence for each narrative incident: denial letters, rejection notices, emails (at least one item per incident)

6 Narrative, PNW, and Declarations

  • Personal narrative: typed (Word or PDF), authored, signed, and dated by the disadvantaged owner
  • Personal Net Worth (PNW) statement using the official USDOT PNW form only
  • Supporting financial documentation for every figure on the PNW statement
  • Signed Declaration of Eligibility for each disadvantaged owner
PNW note: The current net worth cap is $2,047,000. Your ownership interest in the applicant firm, equity in your primary residence, and qualified retirement accounts (401k, IRA, pension) are excluded from the calculation. Verify the current cap at transportation.gov/DBEPNW.
